Implantium Network Terms and Conditions
Implants
The Implantium Network is an innovative way to help all parties: patients, dentists and Implantium. It is a condition of the Network that only Implantium implants are used to treat patients introduced by the Network, either directly or indirectly. Implantium will monitor implant usage and reserve the right to withdraw a dentist from the Network if, in their sole opinion, the dentist is not using Implantium Implants.
Lab Work
Dentists are free to choose which lab they wish to work with. However, Implantium can provide a list of Titanium Implantium Network labs who have agreed to charge a set amount. List available on request.
Booking Process
- Patient will contact Implantium Network
- Implantium Network will send patient details to dentist
- Dentist to contact patient directly to book consultation / further treatment
In the event that any patient contacts a dentist directly and requests treatment in connection with the Network or as a result of the Network marketing activities the dentist must gain the written consent of the patient to pass their information onto the Network and then pass the data on. In the event that the patient is unwilling to give such consent the dentist either withdraws from treating the patient or withdraws from the network. Failure to comply with this rule will result in the dentist being removed from the network.
Fee Structure (charges to patient)
£100 Consultation fee (to include radiographs, but excluding CT scans)
£1350 Per single implant (placement and restoration)
£100 Per extra pontic
£2220 Implant retained denture using two implants (lower)
£3640 Implant retained denture using four implants (upper)
Costing examples:
For every implant that is placed a charge of £1350 is made and per extra pontic required an additional £100 is charged. For example:
A bridge replacing two teeth, using one implant - £1450
(single implant £1350 + extra pontic £100 = total of £1450)
A bridge replacing three teeth, using two implants - £2800
(two single implants £1350 x 2 = £2700 + extra pontic £100 = total of £2800)
Exclusions
- Bone grafting
- Extraction
- Temporary replacement teeth
- Diagnostic wax up
- Dental bars to retain dentures
- Cobalt chrome partial dentures and framework
- Sinus lift
- Complex cases
- Anything else
Costs for treatments considered as an “exclusion” to be set by dentist. A written treatment estimate to be provided to the patient.
Feedback
Feedback is required from ALL patients. Implantium will send the patient a feedback form to complete and return to Implantium. If feedback from patient is not received we will request the support of the dentist in obtaining this information. Failure to obtain feedback may result in temporary or permanent removal of the dentist from the network.
If the feedback received is negative and considered detrimental to the Implantium Network dentists may be removed from the network provider list. The feedback will be investigated but the decision of the Implantium Network is final.
Both the dentist and Implantium are free to withdraw from the plan at any time without notice.
