If you would like to register with the Implantium Network, it's easy!
Just complete the following steps and we will process your application.

1 - Download and complete the Registration Form.
You will need to print off this form, complete in full, sign where appropriate and post back to us at the address below.

If you would like us to post a Registration form to you, please and remember to include your full name and postal address.

2. Agree to our Terms and Conditions.
In order to join the Implantium Network you will also need to agree to our Terms and Conditions, please download this document, sign and and post back to us at the address below.

We aim to process your application within a week and will acknowledge by e-mail unless you ask us to respond differently. If you have not heard from us after a week please ring 0845 3096 322.
